2026 Sports Physicals
- No appointment needed.
- Immunizations will not be given.
- Cash or check only will be accepted.
- Please make checks payable to Sarah Bush Lincoln Fayette County Hospital
- Insurance will not be billed, nor will insurance co-pays be collected.
- Parent or legal guardian must present and complete the medical history forms.
- These physicals DO NOT meet the kindergarten, 6th or 9th grade school physical requirement.
- All fees collected will be donated the school’s sports booster clubs in Bond, Fayette, and Marion counties
All physicals take place at the SBL FCH Walk-In Clinic, 660 W. Taylor St.
